MZG Counseling is in-network with the following insurance plans:

We are out of network with all other insurance carriers and are happy to provide you with a superbill upon request. Our superbills provide all the information required for insurance reimbursement for our services. Please contact your insurance provider to learn more about your out-of-network mental health benefits.

Out-of-Network Fees
The fee for the first session - initial intake is $220
The fee for follow-up sessions is $200
All fees must be paid at the time of service. All sessions are 50 minutes.

Payment
We accept major credit cards. All clients are required to have a credit card on file.
Your co-payments, self-pay fees and any missed session fees are automatically billed to your credit card.

Cancellations
An appointment is reserved for you. If you must cancel an appointment, you must call the office at least 48 hours in advance to avoid a missed session fee. The fee is $200 and will be billed directly to the credit card on file, since insurance companies do not cover these costs. A one-time missed session fee forgiveness may be used at your request. After that, any future missed sessions will be billed to you no matter the reason the session was missed.

Good Faith Estimate for Self-Pay Clients
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. The Good Faith Estimate is provided with your initial documentation requirements.
